SIGN IN AND CREATE AN APPLICATION

If your agency has submitted funding applications to United Way since May 2020 an organization account already exists in SurveyMonkey Apply (SMA) and you should:

• Reach out to your agency’s Primary Administrator for United Way’s SMA system. Your Primary Administrator should login using the same email and password they used to create your existing organization account and create the application. The Primary Administrator should also set up any other individuals who will need access to the application as members of the organization account.
• Once an application is created, the Primary Administrator can set themselves up to be the owner of the application or they can assign someone else from the organization to this role. The Primary Administrator can also assign edit/view access for any application to any individual who is a member of the organization account.

If your agency has not submitted any funding applications since May 2020 to United Way using SurveyMonkey Apply (SMA) you should:

• Click the green Register button in the top right-hand corner of the page to create an account before you access the application. The person who creates the organization account (the Primary Administrator) should create the application and also set up any other individuals who will need access to the application as members of the organization account.
• Once an application is created, the Primary Administrator can set themselves up to be the owner of the application or they can assign someone else from the organization to this role. The Primary Administrator can also assign edit/view access for any application to any individual who is a member of the organization account.

If you are not sure if you have an organization account on United Way's SMA system you should:

• Send an email to csstechsupport@uwgt.org indicating the name of your organization (legal name and any public/operating names used by your organization). We will confirm if an account exists for your organization on the site and who the Primary Administrator is for the organization account.
